Hi, I'm Emma. My partner Patrick recently had a liver transplant, and I saw first-hand how much there was to keep track of in those early days — medications, vitals, symptoms, clinic questions — all while dealing with sleep deprivation, fatigue, and brain fog.
I wanted to lighten the load and help him focus on what really mattered: getting better.
I built Transplant Log after seeing his day-to-day needs and hearing what his clinicians wanted to know from him at each appointment. It started as something just for us, but I'm sharing it because I hope it can help other families going through the same thing.
